About This Position
Coordinate comprehensive care for complex clients. Manage care plans, communicate with healthcare providers, and ensure optimal patient outcomes.
Key Responsibilities
- Conduct comprehensive psychosocial assessments
- Develop individualized care and treatment plans
- Coordinate care with physicians, therapists, and community resources
- Monitor client progress and adjust plans as needed
- Advocate for client needs and rights
- Manage documentation and insurance coordination
- Provide counseling and crisis intervention
Requirements
Must Have:
- •Valid driver's license with reliable transportation
- •Negative background check
- •TB test (within 1 year)
- •References from previous supervisors
- •Professional liability insurance (if applicable)
Nice to Have:
- •Current RN License OR Master's degree in Social Work (MSW)
- •Case management certification (CCM) preferred
- •Strong assessment and problem-solving skills
- •Knowledge of community resources and insurance
- •Excellent written and verbal communication
